30 Day Return Policy - If you are not 100% satisfied with your purchase, you can return the product and get a full refund. We will accept any unopened products for return within 30 days of the original purchase. All products must arrive undamaged and have the original seal. Returns accepted on initial purchase only.

Nutritional Client Statement
I hereby attest to the following:
1. I fully understand that Gregory Pouls, D.C. and Maile Pouls, Ph.D., owners and operators of the Health Enhancement Center and Health Enhancement, Inc., are not medical doctors or medical practitioners. They do not diagnose or treat disease, and that I am not seeking their services for medical or chiropractic diagnosis or treatments. I give permission to Gregory Pouls, D.C., and Maile Pouls, Ph.D. to provide for me a biochemical nutritional urine analysis, nutritional consultation and nutritional recommendations based upon the results of this analysis. I understand they may also require additional saliva, hair, and /or stool analysis in order to make proper nutritional recommendations.
2. As their client, the services performed by Gregory Pouls, D.C., and Maile Pouls, Ph.D., whether in person or by mail or phone, are at all times restricted to consultation on the subject of nutritional matters intended for the maintenance of my nutritional health status and do not involve diagnosing, treatment, or prescribing remedies for the treatment of any disease.
3. I understand that it is my constitutional right to decide how I wish to care for the health of my body. Gregory Pouls, D.C., and Maile Pouls, Ph.D. have not suggested that I cease current medical care I am receiving; be it drug therapy, x-ray treatments, chemotherapy, surgery, or any other medical procedures that my medical doctor deems necessary for my health. I understand that it is my responsibility to check with my prescribing doctor or dispensing pharmacist for any contraindications to any substances, for all medications or procedures they have prescribed to me. If I choose to not follow the recommendations made by my medical doctor, I understand that such a decision is my responsibility and will not hold Gregory Pouls, D.C., and Maile Pouls, Ph.D. responsible for any consequences of such a decision.
4. I am here, on this and on any subsequent visit, solely on my own behalf and not as an agent for Federal, State, or Local Governmental agencies on a mission of entrapment or investigation.
5. If this nutritional client is a minor, the parental signature is the approval for Gregory Pouls, D.C. and Maile Pouls, Ph.D. to provide the above mentioned services and products for my child.
HIPAA Privacy Authorization
1. I authorize Gregory Pouls, D.C. and/or Maile Pouls, Ph.D. to use and disclose the protected health information described below to licensed health care professionals affiliated with Health Enhancement, Inc.
2. This authorization for release of information covers the period of healthcare from all past, present, and future periods.
3. I authorize the release of my complete ‘Health Enhancement Consulting’ diet and health history forms, symptom survey, nutritional laboratory assessment, health history forms, symptom survey, lab reports that I provide to Dr.’s Pouls.
4. This nutritional health information may be used by the person I authorize to receive this information for medical treatment or consultation, billing or claims payment, or other purposes as I may direct.
5. This authorization shall be in force and effect until I have completed all nutritional assessments, dietary and nutritional supplement protocols.
6. I understand that I have the right to revoke this authorization, in writing,at any time. I understand that a revocation is not effective to the extent that any person or entity has already acted in reliance on my authorization or if my authorization was obtained as a condition of obtaining insurance coverage and the insurer has a legal right to contest a claim.
7. I understand that my treatment, payment, enrollment, or eligibility for benefits will not be conditioned on whether I sign this authorization.
8. I understand that information used or disclosed pursuant to this authorization may be disclosed by the recipient and may no longer be protected by federal or state law.
